What To Bring to Lake Forest Clinics

BE PREPARED TO VISIT OUR Oxon Hill OR Gaithersburg OFFICE!
What do you need to bring?

1. Proof of ID
   Government-issued photo ID such as your State-issued Driver’s License or Commercial Driver’s License; State-issued certified copy birth certificate; Non-expired U.S. Passport or Resident Alien Card.
2. Corrective Lens and/or hearing aids
   If you wear eyeglasses or hearing aids, remember to bring them!
3. List of Medications
   If you are prescribed any medications for any condition, including over-the-counter medications, you must bring a list of each medication and dosage, along with the prescribing physician’s name.
4. Previously issued medical card (if applicable)
   If you do not have one or have misplaced it, that is okay. You will be asked your USDOT/FMCSA medical certificate ever been denied or issued for less than 2 years. A dishonest answer may cause the USDOT to cancel your DOT medical exam certification.
5. Records
   Medical test and clearance letter from doctor/specialist (if needed).
6. CPAP machine 90-day compliance report (if needed)
7. A NEED TO PEE!!!
   Drink plenty of water
8. Limit certain foods and drinks
   LIMIT caffeine and salty foods up to 2 hours before your exam: especially if you are dealing with blood pressure issues.

Do You Have A Medical Condition?
What do you need to bring?

Diabetes: Bring a list of medications and name of prescribing doctor. If you are taking these medication: Invokana, Incokamer, Farxiga, Xigduo XR, then a bring your blood labs within the past 30 days!

Heart Condition: (i.e. heart attack, stents pacemaker) Bring in a stress test within the last year and a release from your cardiovascular doctor.

Sleep Apnea: Bring in a copy of your most recent test.

CPAP: Bring a compliance report for the past 90 days. You will need to take your chip from the machine to a sleep center to get the report. If you do not know where to go, call your office and we will help you!
